Commonwealth Home Support Programme (CHSP)

The Commonwealth Home Support Programme (CHSP) supports older Australians to maintain their at-home independence and important connection to the community for as long as possible. It can also support carers of older Australians by providing opportunities for respite.

Support provided at home is for entry-level needs, focusing on daily living requirements. CHSP services are funded by the Australian Government. 

Programme eligibility is decided by the Regional Assessment Service (RAS). As consumer/participants care needs become more complex greater levels of support are required, consumer/participants may progress to a Home Care Package (HCP). 

The CHSP offers a range of services that consumers/participants can stop and start in accordance with their changing needs. Consumers/participants only contribute toward the services they use. 

What services are available as part of CHSP?

Your Commonwealth Home Support Programme can be used for products and services that support your independence at home, your health and well being and your social connections in the community. Services may include:

Personal care
Domestic assistance
Social support
Respite care
Allied health
Transport

How to access CHSP services

Contact Multicultural Care’s Aged Care Connector on 02 9718 6199 (or call My Aged Care directly on 1800 200 422) to discuss your ability to cope and function at home independently, your needs and desired level of support. Multicultural Care’s Aged Care Connector can assist you with accessing aged care support through My Aged Care.   

Our Aged Care Connector can visit you at home to assist you in registering with My Aged Care (for free).

Receive a free home support assessment through RAS (Regional Assessment Service) to determine your needs. At this point you can nominate Multicultural Care as your preferred service provider.

A personalised client record is created for you based on your assessment by RAS. This client record is created to share your information (based on your consent) with your assessor, representative and service providers.

Multicultural Care arranges with you the service delivery particulars, including how many times the service will be delivered, any language or cultural requirements and a clear explanation of the contribution cost for the service (based on an hourly rate).

Your service commences and a monthly invoice is sent to show the contribution fee. You can pay by direct debit or electronic fund transfer.
